SKILLED NURSING
A team of Registered Nurses assess the need of the patient, develops and initiates
the plan of care.  Provides education and consultation with the patient and family,
coordinates other services with other disciplines and supervises the care.  Our
team of Licensed Vocational Nurses supplements the nursing care to patients
under the supervision of the RN and the Physician.  

Skills include, but not limited to:
IV Therapy  *  Wound Care  *  Catheter Care  *  Teaching and Instruction  *  
         Trach Care   *  Anodyne Therapy  * Telehealth monitoring
THERAPY SERVICES

A team of Certified Therapist assess the need of the patient, develops and
initiates the plan of care.  Our team of qualified Registered Therapy Assistants
work under the supervision of the Therapist all working to increase the
patient's function and independence.

Physical Therapy  *  Occupational Therapy  *  Speech Therapy
SOCIAL SERVICES

Provides short-term individual counseling, community resource planning, and crisis
intervention.

Master Social Worker
HOME HEALTH AIDE SERVICES

A team of qualified Home Health Aides, cleared through criminal history
checks and the EMR/NAR,  provide care to the patient under the supervision of the
Registered Nurse.  All Home Health Aides are evaluated every 2 weeks by the
supervising Registered Nurse.

Duties include, but limited to:
Personal Care *Tidying of patient's immediate living area *Meal Prep
                             Home Health Division

Our Home Health Division promotes independent
functioning, prevention of  institutionalization,
and good health for each of our patients.
