Hospice Division
SKILLED NURSING SERVICES

A Team of Registered Nurses assess the needs of the patient, develops
and initiates the   plan of care.  Provides education and
consultation with the patient and family, coordinates other services with
other disciplines    and supervises the care
being provided.
SOCIAL SERVICES

Provides individual counseling, community resource planning, and crisis intervention.
SPIRITUAL SERVICES

Provided spiritual counseling/guidance for the patient and family.  
HOME HEALTH AIDE SERVICES

Services are provided by a qualified and  experienced Home Health Aide.  Stringent         
    background checks are performed including the EMR/NAR.

Personal Care * Reporting changes to RN supervisor *
light meal prep *  tidying of patient's immediate living area
PHARMACY / MEDICAL EQUIPMENT / SUPPLY SERVICES

Patient receives all medication, medical equipment, and daily supplies
pertaining to the terminal diagnosis at
NO COST.

VOLUNTEER SERVICES

Volunteer Services are provided to patients in need of a little additional
help.  All volunteers are trained and orientated to the hospice concept.
Stringent background checks and verification through the EMR/NAR
are performed.

Hospice helps the whole family cope
with the fear of the inevitable (death),
being in pain, being a burden,
and being alone by providing a team of
Skilled Nurses, Social Workers,
Chaplains, Home Health Aides, Physicians,
and Volunteers.
